Salt Lake City: Jazzamin Clark Charged After Utah Medicaid Paid $4M for Therapy That Allegedly Never Happened

Utah Medicaid paid more than $4 million for therapy claims that federal prosecutors say were not legitimate. Because apparently paperwork is now a treatment plan.

Jazzamin Clark was charged with health care fraud, wire fraud, and money laundering. Prosecutors allege she submitted claims for therapy that was never provided, was performed by unqualified or unlicensed people, or was not covered at all.

This is an allegation, not a conviction. But the bill already landed on taxpayers.
